Your Responsibilities:
Conduct comprehensive assessments, including annual wellness exams, in patients' homes and virtually.
Provide patient and family education on available benefits and programs to promote healthier living.
Maintain accurate and detailed patient records, including medical history, diagnoses, and treatment plans.
Coordinate care with primary care providers, specialists, and ancillary services.
Deliver timely, evidence-based care to minimize hospitalizations and enhance patient quality of life.
Prescribe medications, order tests, and collaborate with physicians to develop and implement treatment plans.
Educate patients on preventive monitoring and schedule diagnostic testing to close gaps in care.
Assist patients with accessing educational resources.
Participate in integrated care team meetings.
Make assessments of patients health status, develop treatment plans, and implement and evaluate those plans.
Other duties as assigned.
What We're Looking For:
Active and unrestricted Registered Nurse and Nurse Practitioner (NP) or Physician Assistant (PA) license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Proficiency with medical instruments, equipment, and computer-based data management systems.
Excellent verbal and written communication skills.
Strong understanding of HIPAA and OSHA regulations.
Minimum of 2 years of direct patient care experience.
Experience in managed care, IPA, or health plan settings is preferred.
Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home is highly valued.
Bilingual in English and Spanish is highly preferred.
