Substance Use Counselor at AppleGate Recovery summary:

Licensed counselor provides individual and group counseling, assessments, and treatment planning for adults receiving Medication Assisted Treatment for opioid use disorder using evidence-based, trauma-informed approaches. Collaborates with multidisciplinary team members to evaluate progress, coordinate services, develop aftercare/discharge plans, and respond to patient concerns. Requires a relevant master’s degree and state licensure (LCAC, LAC, LMHC, or LCSW), background check and drug screen; position is full-time with benefits.

Primary Job Function:
The licensed counselor will be responsible for offering support and assistance to patients enrolled in Medication Assisted Treatment for opioid use disorder. Utilizing evidence-based practices and a trauma-informed approach, the counselor will help patients develop indi

Indi vidualized coping techniques and symptom management skills that enable then to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.

Essential Job Functions:

• Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
• Conducts initial assessment and evaluation to determine eligibility for services.
• Complete Bio-psychosocial assessment.
• Establish individualized treatment plan with each patient and review as clinically indicated.
• Respond, as needed, to patient grievances and complaints.
• Develop aftercare and discharge plans.
• Other related duties as determined by supervisor.

Qualifications:

• Must be a licensed in the state of INDIANA Licensed Clinical Alcohol Counselor (LCAC) , Licensed Addiction Counselor ( LAC ) , Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW)
• Must have a Masters degree in a qualifying field. 
• Satisfactory drug screen and criminal background check.
