Clarity HR Solutions is partnered with Project IMPACT to find passionate and qualified In-Home Outreach Counselors to join the Project IMPACT Family Preservation Team. This position is part-time at 17.5 hours per week.

About the Position
The In-Home Outreach Counselor is responsible for providing in-home case management duties through the Family Preservation Program which include:
- Provide in-home outreach counseling, initial assessment, and case management services to assigned clients weekly.
- Orient clients to program philosophy, expectations, services available, treatment process, rules/regulations, client rights, and other related information.
- Required to observe family environment to ensure the safety of children in-home setting.
- Develop individualized service plans based on assessment reports provided.
- May conduct individual and family sessions as required to meet the needs of the family.
- Evaluate parent/child/family progress and provide progress letters to LA County Social Worker (CSW).
- Communicate observed behavior, progress and/or issues with LA County Social Worker (CSW), Program Manager, and/or Clinical Supervisor.
- Work with staff to assist in the identification of community resources/linkages to ensure that families are provided with full range of available resources.
- Arrange transitional planning of family upon completion of services.
- Must participate in MCPC (Multidisciplinary Case Planning Committee) for assigned clients per schedule.
- Document weekly visits and submit documentation to quality assurance staff according to guidelines.
- Submit weekly and monthly reports to Clinical Supervisor for review.
- Must attend weekly clinical supervision, staff meetings, and in- service/professional development trainings as required.
- Assist in other duties as required.
Job Requirements
- Bachelor's Degree in Human Services or Behavioral Science but a Master’s degree is preferred.
- Proficiency in a data entry computer database and Microsoft Office Suite (Outlook, Word, Excel).
- Minimum of 1 year of case management experience in a social services agency setting.
- Must have reliable transportation to travel while providing case management services to assigned clients. Must provide insurance information.
