Summary

The Case Manager (CM) II is responsible for the leadership and functioning of their assigned case load. Case Managers develop professional and empathetic relationships while providing clients with connections to appropriate resources through one-on-one Case Management that develop individualized case plans that promote client progression towards obtaining and maintaining self-sufficiency. The CM II serves as a subject matter expert, leads the Case Management team when the Supervisor of Case Management is unavailable, and may represent the organization both on-site and throughout the community. The CM II will develop, facilitate and/or co-facilitate life skills activities for the residents at the Nestor Program for a total of 84 hours for the calendar year.

Essential Functions

  • Provides ongoing intensive support to clients which can include assessing, evaluating, and coordinating services, crisis intervention and applications for benefits & referrals to community resources such as but not limited to: Childcare Assistance/Services, Employment & OJT Placement/Training, Education Assistance/Benefits Information, Financial Counseling/Debt Counseling, Legal Services, Domestic Violence Support, Medication Management, and Representative Payee.
  • Manages a case load of clients and carries a higher caseload of clients during periods of low staffing to support the ongoing operation of services; and enters, monitors and updates client database.
  • Prepares, presents and documents client cases and participates as a member of a multidisciplinary team that prepares and presents client case presentation.
  • Communicates and advocates to landlords to support clients in maintaining their housing.
  • Plans, coordinates and facilitates client orientation, meetings and events.
  • Transports clients, as needed to medical appointments, income related appointments, referral services, and other as needed services critical to client case plan progress and housing stability.
  • Supports team in identifying valuable education topics, strategies, community resources and occasionally leads trainings that can be incorporated into services.
  • Develops and implements a plan to provide social events and community engagement opportunities to increase resident engagement; develops flyers.
  • Collaborates with service providers to develop a site program schedule for residents with access to a variety of recreational, educational, and enrichment activities in support of resident needs.
  • Assists supervisors with onboarding process of new hires including, but not limited to interviews, trainings, and shadowing.
  • Stands in the absence of the Supervisor of Case Management and assists with file reviews.
  • On time completion of assigned training and policies.
  • Performs other duties as assigned.

Qualifications

  • Bachelor’s Degree from an accredited four-year college or university with major coursework in healthcare, psychology, sociology, social work, public administration, or a related field, and one (1) year of applicable job experience, or
  • Associates Degree with a focus on healthcare, psychology, sociology, social work, public administration, or a related field and two (2) years of applicable job experience, or
  • High school Diploma or GED and a minimum of a combined two (2) years of lived experience, and at least two (2) years of applicable job experience
  • Demonstrated basic understanding of principles of Housing First, Trauma Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations and Prevention and Diversion practices, as well as theory, principles and practices of homeless housing interventions, social services, case management, eviction prevention, shelter diversion, and crisis intervention.
  • At least 2 years’ experience in case planning and/or completing assessments in a social service setting.
  • Basic user of MS Office.
  • Ability to pass fingerprinting and background check upon employment.
  • Must have a valid California Driver’s License and be able to qualify for insurance coverage
  • Participate in an annual Tuberculosis screening and/or other screenings when necessary. Basic user of MS Office
